By agreeing to these terms and conditions, you, the client, hereby understand and acknowledge that the training, programs, and events held by Unlimited Jiu Jitsu, LLC ("the Company") may expose you to inherent risks, including accidents, injury, illness, or even death. You assume all risks of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks known and appreciated by you.

You hereby acknowledge your responsibility in communicating any physical and psychological concerns that might conflict with participation in the activity. You acknowledge that you are physically fit and mentally capable of performing the physical activity you choose to participate in.

After having read this waiver and knowing these facts, and in consideration of acceptance of your participation and the Company furnishing services to you, you agree, for yourself and anyone entitled to act on your behalf, to HOLD HARMLESS, WAIVE AND RELEASE Unlimited Jiu Jitsu, LLC, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of your participation in Unlimited Jiu Jitsu training, programs, and/or events.

Unlimited Jiu Jitsu, LLC recommends all clients seek the advice of a physician prior to engaging in any physical activity in our studio. By signing this document, you confirm you have been cleared for vigorous physical activity by a physician.

You give Unlimited Jiu Jitsu, LLC the right to display pictures, video, or any media with yourself present in any way. If you decide to opt out of this, you agree to submit a letter of exclusion in writing to Unlimited Jiu Jitsu, LLC.

Arbitration Agreement: Any dispute arising out of or related to this agreement or your participation in Unlimited Jiu Jitsu training, programs, and/or events shall be resolved exclusively through binding arbitration in accordance with the Federal Arbitration Act and the Commercial Arbitration Rules of the American Arbitration Association. The arbitration shall take place in the State of Nevada.

By your signature, you indicate that you have read and understand this Waiver of Liability. You are aware that this is a waiver and a release of liability and you voluntarily agree to its terms.
